There will be occasions when this group
may travel to a bowling alley, the YMCA, the FHS tennis courts, Meadowlark Park,
a golf course, Portage Park, etc. to participate in activities out of the
school. Students will be transported by
bus. We may also go for walks off school
property or walk to the city reservoirs to go canoeing. These events may not happen as scheduled due
to weather conditions or realignment of the school schedule. As this student’s parent or guardian, I
release the School, Catholic Youth & School Services, and any associated
person or agency from any claims in consideration for the opportunity to
participate in this program. Please fill
out the section below, as well as the back, and have your student return the
form to me immediately.
